2024 International Shoulder Group Conference - Student On-Campus Accommodations Form

Please complete this form if you would like on-campus accommodations in the Pond Road Residence two-bedroom suites (https://www.yorku.ca/conference/keele/accommodation/groups/pond-road/).

These rooms are based on double occupancy. You will be asked to list the name of the person you are sharing a room with. Both roommates will need to complete this form.

Please do not complete this form until you have found a person to room with. If you would like our assistance with trying to pair you with a roommate, please contact isg2024@yorku.ca prior to completing the form.

You will be asked to fill in your order number and amount paid on this form. Therefore, please complete payment following the 2024 International Shoulder Group Conference Student Accommodations Eventbrite link found on the conference webpage (www.yorku.ca/events/isg2024) before filling out this form.

The rate for double occupancy (if you would like the two-room suite to yourself) is $138.36 CAD per night including taxes and fees. The rate for single occupancy is $69.82 CAD per night including taxes and fees. You must have a roommate for you each to pay the single occupancy rate. Rooms have been reserved for June 3, 4 and 5.

NOTE: Please do not complete this form if you are faculty/staff booking a room in the Executive Learning Centre. Please see our website for instructions for this booking.
