Welcome to the CCSC Monetary Awards Nomination Form for the 2024/2025 term.

Calumet and Stong Colleges (CCSC) annually recognizes the outstanding contributions and achievements of students through monetary awards to affiliated students.

Please read the following instructions carefully.

You have been requested by an applicant to act as their reference by completing this nomination form. Nominator forms must be submitted by January 13th, 2025, 11:59pm to be considered.

Candidates are being assessed based on their contributions to the colleges from the period of Fall Term 2023 to August 2024. Please ensure you are speaking on their experiences prior to August 2024 in this nomination form, otherwise your nomination will be ineffective.

You may complete multiple nomination forms for more than one candidate at your discretion. Please note if you refresh this form, you will lose your progress.
